Team Work Gets Patient Back to Health and Back to Home Again
Margaret Brace came to Holt Senior Care & Rehab Center during the summer of 2012 after a hospital stay. She was experiencing falls, health concerns, and was fearful of being home alone. She needed 100% assistance with housekeeping, meal prep, laundry and medications. It was felt at that time that supervised living arrangements and long term placement were needed.
Margaret showed significant declines initially due to her medically complex conditions. She had to take very small steps in order to maintain her balance and could only walk 15 feet with assistance. During the course of Margaret’s stay, the clinical and rehabilitation teams at Holt Senior Care & Rehab Center were able to help improve Margaret’s medical conditions and she began making drastic improvements. Her strength and balance gradually improved to the point where Margaret was able to move independently about the center.
Margaret’s joy of caring for others kept her motivated to get back home. She has always enjoyed cooking, cleaning and taking care of her family. We were excited that in May 2013 (less than a year) Margaret made a complete turn around and was able to discharge from long term care. She now lives successfully with her daughter and granddaughter and couldn’t be happier.
